The designation Q was first used on newly-arrived R27 class equipment in 1960 to describe the service of the Brighton Beach Express (No. 1 in BMT nomenclature). This service was similar to the current Q service, except that it ran express in both Brooklyn and Manhattan between approximately 6 am and 9 pm on weekdays, and terminated at Brighton Beach rather than Coney Island.

At the same time, using the Independent Subway System protocol of using double letters to designate local trains, the Brighton Beach Local was signed as QT or QB, the first being used for daytime trains that operated through the Montague Street Tunnel to Manhattan, and the latter used for locals that operated over the Manhattan Bridge when the Q did not run. The current Q service is virtually identical to the former QB service, though today it runs at all days and hours.

When double letters were dropped, the Q ran between 57 St/7 Ave, Manhattan and Coney Island, Brooklyn during rush hours, making express stops on the BMT Broadway Line in Manhattan and local stops on the Brighton Line in Brooklyn. When the north side of the Manhattan Bridge was closed in 1986, Q ran skip-stop with the D in Brooklyn.

In 1988, the north side reopened and the south side was closed. Q trains became the weekday Brighton Express and was rerouted over the north side of the bridge and via 6 Ave to 57 St/6 Ave (to 21 St/Queensbridge in 1989). During the evening hours, a shuttle ran between 57 St/7 Ave and 2 Ave. This was replaced by B service in 1990.

In May 1995, the north side of the bridge was closed during midday and weekends. During this time. Q trains ran local in Brooklyn and then via the Montague Tunnel to Canal St on the Broadway Line. From there, it ran express to 21 St/Queensbridge.

On July 22, 2001, the north side of the Manhattan Bridge was closed while the south side reopened. There were two Qs. The circle Q ran local in Brooklyn and terminated at Coney Island while the diamond Q ran express in Brooklyn and terminated at Brighton Beach. Both Qs then ran via the south side of the bridge to Canal St and then express on the Broadway Line to 57 St/7 Ave.

After September 11, 2001, the Q local replaced R trains between Canal St and Forest Hills/71 Ave at all times except late nights, when it terminates at 57 St/7 Ave. Service went back to normal on October 28.

On September 8, 2002, Coney Island/Stillwell Ave was closed for rehab and the Q local was cut back to Brighton Beach. It returned to Coney Island on May 23, 2004.

From April 27-November 2, 2003, the south side of the bridge was closed on weekends and Q service was rerouted via the Montague Tunnel during this time.

On February 22, 2004, the north side of the Manhattan Bridge reopened. The Q Diamond was discontinued and was replaced by the B in Brooklyn and N in Manhattan.
